New Patients
Register as New Patient
You will need to complete a practice new patient questionnaire, as well as our registration forms. These are available at reception.
We will ask you for proof of identity which will include evidence of address (such as a recent utility bill) and one form of photographic ID (please select the link to refer to our 'registration policy').
Once you have been accepted as a patient your medical records will be transferred to the practice and you will be sent a new medical card. Your medical records may take several weeks to arrive at the Practice.
If you are on regular medication, you will need to get a summary printout from your previous GP practice. This will reduce any delay on ordering your medication. Failure to provide a summary printout from your previous GP practice will cause a delay with your medication being issued as your new GP will not have the information available to add new medication to your record.
We advise all our new patients to make a routine health check with our practice nurse within the first three months of joining our practice.
The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area. If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.
We can only accept new registrations from patients who live within a 6 mile radius of Ballyowen Health Centre.
Change of Personal Details
Please make sure your contact details are up to date by informing our reception team of any change in address or telephone number. This will reduce the risk of delay in contacting your regarding your health.
Online Services
Please contact the receptionist to express your interest in availing of the online prescription ordering service. The practice is currently testing the system to ensure smooth use of the service for patients.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days (short stay) to 30 days (long stay). After 14 days you will need to register as a temporary or permanent patient.
You should be able to provide a list of any medication you are currently taking. This will help reduce any delay in prescribing medication for you.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
